Download the claim form or collect one from any of our offices
Step 3 - Report
Report to the Police or nearest Police Station, in the event of accidental injury or death
Step 4 - Report
Please report the incident to the Labour Department office immediately

Checklist
- Photocopy of BUR555/BUR500
- Photocopy of Identity Card/Passport of injured worker
- Photocopy of Form A (to the Labour Department)
- Original Medical Bills and Medical Report
- Every Letter, Claim, Writ, Summons and Legal Proceeding, should be notified or forwarded to National Insurance Company Berhad immediately upon receipt
